Eglinton Way Dentistry — Privacy Policy
Last updated: October 17, 2026
Your privacy is an important part of your care. Eglinton Way Dentistry respects the confidentiality of your personal information and personal health information.
This policy explains how we collect, use, disclose and protect information when you receive care, visit our website or communicate with us in person, by telephone, email, text message or other channels.
We handle personal health information in accordance with Ontario’s Personal Health Information Protection Act, 2004 (PHIPA), applicable professional requirements and other applicable privacy laws, including the Personal Information Protection and Electronic Documents Act (PIPEDA) where it applies.
Information we collect
We collect information reasonably necessary to provide dental care, communicate with you and operate our practice. This may include:
Your name, date of birth, address, telephone number, email address and emergency contact.
Medical and dental history, medications, allergies, examination findings, treatment plans, clinical notes, X-rays, photographs, scans and other clinical records.
Insurance, benefit-plan, billing and payment information.
Consent records, appointment details, communication preferences and correspondence.
Information you provide through website forms or other communications.
Technical information about website visits, such as IP address, browser and device information, pages viewed and interactions with website content.
We generally collect information directly from you or your authorized representative. We may also obtain relevant information from other health-care providers or organizations with your consent or as permitted or required by law.
How we use and share information
We use information to assess your needs, provide and coordinate care, maintain records, arrange appointments, communicate about treatment, process payments and benefit claims, respond to inquiries, and meet legal and professional obligations.
Where appropriate and legally authorized, we may share relevant information with other treating health-care providers, dental laboratories, insurers, benefit-plan administrators and service providers supporting our practice.
Service providers acting on our behalf are authorized to handle information only as needed for their services and subject to applicable confidentiality and privacy requirements.
We may also use or disclose information without consent where the law permits or requires it, including for mandatory reporting, certain regulatory activities or responding to a legally valid demand. We limit information handling to what is reasonably necessary for the purpose.
Your consent and choices
Depending on the circumstances, consent may be express or implied. We may rely on implied consent for care-related purposes where PHIPA permits it. We obtain express consent where required.
You may ask questions, withhold consent or withdraw consent for future uses or disclosures, subject to legal and professional limits. We will explain if a restriction affects our ability to provide care or administer your benefits. Withdrawing consent does not undo information handling that has already occurred or prevent recordkeeping required by law.
We respect applicable consent and capacity rules when dealing with children, family members and authorized representatives. A family relationship alone does not automatically authorize access to another patient’s information.
Communications and promotional messages
We may contact you about appointments, recalls, treatment, billing and other matters relating to your care. Please tell us if your contact information changes or you have preferences about how we communicate.
Promotional emails and text messages are sent only with the consent required by applicable law. Where personal health information is used for marketing, we obtain express consent. You may withdraw marketing consent by using the unsubscribe instructions in a message or contacting our office. Declining promotional messages does not affect your access to dental care.
We obtain specific consent before using identifiable patient photographs or other patient information in marketing.
Ordinary email, text messages and website forms may carry privacy risks. Please avoid sending detailed health information through general website forms or ordinary email. Contact our office to discuss an appropriate way to exchange sensitive records.
Website privacy
Our website may use cookies and similar technologies to support functionality, understand website usage and measure advertising performance. Website tools and embedded third-party features may collect information about your device and browsing activity.
You can manage cookies through your browser and any privacy controls provided on the website. Disabling some cookies may affect functionality. External websites and services have their own privacy policies.
Protection and retention
We use reasonable administrative, physical and technical safeguards to protect information against loss, theft, and unauthorized access, use, disclosure or alteration. Access is limited according to authorized responsibilities. No method of electronic communication or storage is completely secure.
We retain records for the periods required by law and professional standards and for other lawful purposes. When information is no longer required, we arrange for secure disposal appropriate to the record. Requests to delete information remain subject to mandatory retention requirements.
If a privacy breach occurs, we take appropriate steps to respond and provide notifications required by law.
Access, corrections and concerns
You may request access to your personal health information, request corrections or ask for records to be transferred to another provider. We may verify your identity and explain any applicable limits or permitted fees before proceeding.
For privacy questions, requests or complaints, please contact our office and indicate that your inquiry concerns privacy:
